Vision
Clear vision helps make it possible to fully enjoy life’s little moments. Taking care of your eyesight with regular checkups and the right glasses is a simple step that makes a big difference in overall well-being. We've partnered with United Vision Plan to provide your glasses and/or contacts. Just go to an in-network provider to obtain your vision test and prescription, then “try on” your glasses virtually in the comfort of your home on your phone, computer, or iPad.
How It Works
You can visit United Vision Plan’s website listed below to:

Look: United Vision Plan’s member website allows you to use a picture of a model, upload a selfie, or your camera to access a live mirror view.

Upload your prescription and order: once you decide on what item you want, you can order.

Note - all online eyeglass companies require a PD (Pupilary Distance) measurement. This is usually on your prescription, but you may want to ask your eye doctor at the time of the exam to include it. If you don’t have it, United Vision Plan's member portal will also provide instructions to measure yourself, if not available.

Annual Allowance: You have an allowance for glasses and contacts. The website will show your costs for each item selected, after benefits are applied.

Depending upon the Elite Health Plan coverage you have selected, you will receive either a $250 (Signature Plan) or $300 (Core Plan) general allowance per year toward your eyewear materials.

Your benefit can be applied to the purchase of either 1 pair of glasses or 1 contact lens supply transaction per calendar year.

Your glasses or contacts should ship directly to you within several days. If you are not completely satisfied within the first 14 days after delivery, you can return for either a remake or have your benefits reset to select another pair of glasses. Simply call the customer service number below.
