Part B Diabetic Supplies

Medical Supplies associated with the injection of insulin

Medicare Part B covers some diabetic test supplies, including blood glucose (blood sugar) test strips.  Medicare Part B covers  lancets and lancet holders. For Lancets, every three (3) months, you will be able to get up to 300 lancets if you use insulin or 100 lancets if you do not use insulin.   It is important that the supplier is registered and enrollment to participate in Medicare.  

Continuous Glucose Monitors (CGM):

If you have diabetes, Medicare may cover a continuous glucose monitor (CGM) and related supplies if your doctor or other health care provider prescribes them for you and you meet the following conditions: 

  • You take insulin or have a history of problems with low blood sugar (hypoglycemia).

    • Recurrent (more than one) level 2 hypoglycemic events (glucose <54mg/dL (3.0mmol/L)) that persist despite multiple (more than one) attempts to adjust medication(s) and/or modify the diabetes treatment plan; or,

    • A history of one level 3 hypoglycemic event (glucose <54mg/dL (3.0mmol/L)) characterized by altered mental and/or physical state requiring third-party assistance for treatment of hypoglycemia

  • Your health care provider has decided that you or your caregiver have had enough training to use a continuous glucose monitor as prescribed. 

  • Within six (6) months prior to ordering the CGM, your physician has an in-person or Medicare-approved telehealth visit with you to evaluate their diabetes control and determined that criteria are met.

  • The following are our preferred Continuous Glucose Monitor(s):

    • Dexcom G6, Dexcom G7, and Dexcom G17 15 day

    • FreeStyle Libre, Libre 2 Plus and 3 Plus Sensor.

  • This does include the receiver, transmitter, reader, and sensor.

Blood Glucose Meters (BGM):

To be eligible for coverage of home blood glucose monitors and related accessories and supplies, you must meet both of the following criteria:

  1. You have diabetes; and,

  2. Your healthcare provider has concluded that you have sufficient training using the blood glucose meter for the frequency of blood glucose testing.

 

Lancets, blood glucose test strips, glucose control solutions and spring powered devices for lancets are covered if the glucose monitor is covered. Lancet devices are limited to one every 6 months. Blood glucose test strips are covered by Medicare guidelines according to insulin administration to determine the applicable quantity per time frame limitation.

  • Manufacturer: Abbott Diabetes Care

    • Products: Freestyle Insulinx, Freestyle Lite, Freestyle Precision, Freestyle, Precision Xtra

  • Manufacturer: Ascensia Diabetes Care

    • Products: Contour, Contour Plus, Contour Next

Medical Supplies associated with the injection of insulin

According to CMS regulations, medical supplies associated with the injection of insulin are covered under Medicare Part D, or the drug benefit program.  This includes syringes, needles, alcohol swabs, gauze, and supplies that are directly associated with delivering insulin into the body.   Insulin delivery devices not otherwise covered by Part B, such as insulin pens, pen supplies, and need-free syringes also are included in this definition.    However, if Elite Health Plan is not able to reasonably determine the presence or validation of insulin usage, these products may be covered under Medicare Part B if being utilized for purposes outside of insulin.   For Part D coverage, please consult the Elite Health Plan formulary. 

How to obtain a Blood Glucose Meter at your local pharmacy:

As an Elite Health Plan Member, you are able to obtain a Preferred Blood Glucose Meter in multiple ways noted below in the flyers attached below.​

 

1. Call the number specified to request a meter be shipped to your home.

2. Access the website form and complete the requested information to have a meter shipped to your home.

3. Print off the flyer and take it to your local network pharmacy to obtain a meter. 


You will still need a prescription from your healthcare provider and to meet the applicable Medicare coverage requirements.    

You may simply print the flyer off and follow the directions in the flyer or simply take the flyer to your local pharmacy.
