Personal Emergency Response System (PERS)
Elite Health Plan is excited to offer members a Personal Emergency Response System (PERS) through Medical Guardian. This benefit is ONLY AVAILABLE ON THE SIGNATURE PLAN.
A PERS device provides peace of mind for you and your loved ones by giving you immediate access to help in case of an emergency, whether it’s a fall, sudden illness, or other urgent situation.
How It Works

Your doctor will need approval from Elite Health Plan before this service can be provided

Once you are approved:

You’ll be able to search online to see the different device options available

Speak with a PERS representative who can help you choose the right device.

The team will then process the order, and it will be delivered within 7-10 days

Once delivered, your device will be activated and ready to assist if needed

This benefit is available only through the our Signature HMO Plan with approval through a prior authorization.
