Important Message About What You Pay for Insulin

You won’t pay more than $35 for a one-month supply of each insulin product covered by our plan, no matter what cost-sharing tier it’s on.
2027
Elite Health Plan Signature HMO (001) - You won’t pay more than $15, or 25%, whichever is less for a one-month supply of each insulin product covered by our plan.
Elite Health Plan Core HMO (002) - You won’t pay more than $35, or 25%, whichever is less for a one-month supply of each insulin product covered by our plan.
