Special Supplemental Benefits for the Chronically Ill (SSBCI). You may qualify for the healthy food and produce benefit if you have been diagnosed with a chronic condition such as Diabetes, Cardiovascular Disease, Congestive Heart Failure (CHF), Mental Health Conditions, Cancer, or other qualifying conditions listed in your Evidence of Coverage. You must also meet all other eligibility criteria.
If you qualify and are validated to have one of the chronic conditions by your provider, you can use your NationsBenefits® Mastercard® Prepaid Card allowance* to pay for eligible items Purchases can be made at participating retail locations.
*Please check the Evidence of Coverage (located on either the Signature or Core page under plan documents) for details about each specific plan.
How It Works
1. Provide the SSBIC verification form to your provider. You can also request our Care Navigator team to assist.
2. Once we receive a signed verification form from your provider, we will work with NationsBenefits® to allow groceries to be purchased using your prepaid card allowance. (Please allow some time for activation of this benefit). You can always reach out to our Care Navigator team to assist.
3. For a full list of participating retailers, go to EliteHealthPlan.NationsBenefits.com or the Benefits Pro App. You can also check your card balance there as well.
