We are pleased to welcome you to the Delta Dental plan for Elite Health Plan. We want to help you stay healthy by giving you dental care. It’s important to visit the dentist regularly to prevent issues — not just when something hurts. Seeing your dentist often can help stop problems before they start. We do provide you with care when issues may arise.

How It Works 

You can visit the Delta Dental website listed below to:

• You are provided an annual allowance (amount differs based upon the plan you choose) to cover the cost of your dental care within the network.

• Find a Medicare Advantage PPO dentist near you (before you get work done, check to make sure your provided is in in the Delta Dental Medicare Advantage PPO Network)​

• See your evidence of coverage for the routine, preventative, restorative and comprehensive dental services allowed

If you have questions, Delta Dental has a special phone number (see below) for members of Elite Health Plan. You can call them directly to get help. Delta Dental will also send you a separate ID card for you to use when you choose your dental provider and go to their offices.
